Rex Warden has provided a picture of his grave. Rex says:

The grave of Peter Worden V is in a small cemetery in the hills above Cheshire, Berkshire Co., MA. The church where he preached was located there at one time but is long gone. Col. Peter Worden VI is buried next to his father and their gravestones are identical. There is no stone for Mercy but she must also have been buried there. The epitaph, written by him, reads as follows.
Here lies the body of Peter Werden, late pastor of the Church of Christ in Chesire. He was born June 6 1728 converted by the mighty power of God to the Lord Jesus Christ May 8th 1748 In the month of May 1751 he was ordained to the work of the ministry in Warwick and continued measurably faithful in his Pastoral charge to the close of his which was February 21, 1808.
His soul to God he ask to send
To cry for grace for foe and friend
But blessed be the God of Love
His soul is now with Christ above
This crumbling sculpture keep the clay
That used to house his noble mind
      But at the resurrection day
A noble body he shall find

If you look closely at the stone you will notice that there are three different sizes of the lettering. I always wondered if that was planned or if the person who engraved the stone decided he was running out of room and had to size down the lettering.

Rex Warden has also provided a photocopy of his will:
